Requesting Copies of Your Medical Records
You have the right to review and/or request a copy of your medical records. Please check myHealthFile, our patient portal, to see if the information you need is already available to you. myHealthFile, is an easy and secure way to access your information at no cost to you.
You may request copies of your medical record by submitting one of the forms below. If you do not need a copy of the entire medical record, you may want to request an abstract of the record which includes a subset of reports and documents most frequently requested.

There are no in-person requests for medical records. If you have any questions or would like a Release of Medical Information Form sent to you, please call 304-691-1177.
No. Medical information is confidential, and it cannot be released over the telephone.
We make every effort to respond to all requests as quickly as possible. The time to complete each request will vary, but please allow at least five consecutive business days for request completion. We are granted up to 30 days to respond to a request.
Maybe. Under certain circumstances, a copying fee may be involved. Generally, one free copy is given, but you will be notified if a fee will be applied to your request. The fees are based on West Virginia Law and updated annually.
