A record of each serious complaint will be kept for at least 3 years from the date the complaint was received by the Mammography Department. (A serious complaint means an adverse event that may significantly compromise clinical outcomes, or an adverse event for which a facility fails to take appropriate corrective action in a timely manner.) If the complaint is not resolved to the satisfaction of the consumer, the patient or patient representative (such as family members or referring physicians) should mail complaint to the facility’s accreditation body at the following address:  Director, Breast Imaging Accreditation Programs, American College of Radiology, 1891 Preston White Drive, Reston, VA. 20191-4397 or faxed to 1-800-227-5463.
