At Marshall Health, Patient Centered Medical Home (PCMH) is the personalized approach we take to provide comprehensive primary care. This means that Marshall Health is helping meet your physical and mental health care needs, including prevention, wellness, acute care and chronic care, with a designated team of Marshall Health physicians, nurses, pharmacists and more.

Your medical home helps you navigate a complex medical system. By taking a proactive approach to preventive care through timely tests, check-ups and treatment of chronic conditions, enrolling in a Medical Home with us will, hopefully, also keep you healthier.

To better coordinate your care, we also keep track of your visits to other healthcare providers within the Marshall Health network. This includes in-patient care at any of our partner hospitals.

Through this program, Marshall Health offers expanded access to care. Evening hours and new options for online communication make it easier to get the care you need when you need it. You can also communicate non-emergency issues with your health care team via our FollowMyHealth patient portal.

Getting started

Medical home starts with you—the patient—identifying your primary care physician. You can Find a Doctor or work with one of our Medical Home Coordinators to identify a primary care doctor that meets the needs of you and your family. You can find one at more than a dozen locations throughout the region, including Cabell, Putnam and Wayne counties.
